2D vs 3D Gait Analysis: What the Difference Actually Buys You

If you have looked into a running gait analysis, you have run into two versions that sound similar and are not. One is a slow-motion video review. The other is 3D motion capture. Here is what actually separates them, and when the difference matters.
What 2D is, and where it is genuinely fine
2D analysis is video. Someone films you running, usually from the side and behind, and reviews it frame by frame. A skilled clinician can catch real things this way. An obvious overstride, a heavy heel strike, a knee that visibly caves in.
If that is what is going on with you, 2D may be all you need.
Where 2D runs out of road
The limit is not the clinician. It is the format. A camera on one side of you sees one plane. Running happens in three.
RunDNA, the system behind our analysis, says it directly: 2D at a high frame rate can identify mechanics but lacks reliability for most measures, and should be read as qualitative, not quantitative. Meaning you get an impression, not a measurement.
That gap matters most for the things that actually cause running injuries. Pelvic drop, hip rotation, how far your knee travels inward, side-to-side asymmetry. These live in the planes a side-on camera cannot see, and they are exactly what sits upstream of the injuries runners get. The pain shows up at the knee or shin. The cause is usually at the hip, in a plane 2D cannot measure.
What 3D adds
3D motion capture uses reflective markers and multiple cameras to reconstruct your body moving through space. It is the reference standard for this kind of measurement, which is why research labs use it rather than a single camera.
The practical difference is a number instead of an opinion. Not "your hip looks like it drops." Your hip drops five degrees more on the right than the left. That precision changes what happens next. You cannot track progress against an impression, and you cannot tell a five degree problem from a one degree one by eye. With a measurement, the plan targets what is actually there, and you can re-measure later to see if it changed.
So which do you need
Honest answer: it depends on the question.
If you want a quick look and something obvious is going on, 2D can get you there. If you have an injury that keeps coming back, a plateau you cannot explain, or you want to change your mechanics and know whether the change is holding, you want measurement. You cannot fix precisely what you could only estimate.
